Golden thread O&M manual: what it is and what goes in it
The operation and maintenance (O&M) manual tells the people running the building how its systems and equipment work and how they must be maintained. In the golden thread it is the practical link between as-built design and day-to-day safety management.
Why the manual matters
The O&M manual collects the information needed to run the building: how each system works, how it is operated, what maintenance it needs, and who supplied it. It is produced during construction and handed over at completion, and it must keep working for the life of the building as systems are replaced and upgraded.
In the golden thread the manual matters twice: as a record of what was installed (linking to drawings and the asset register) and as the instruction set that keeps safety systems maintained correctly.

What usually goes wrong

Is the O&M manual legally required in the golden thread?

Who produces the O&M manual?
During construction, the principal contractor is responsible for delivering the O&M manual at completion, usually assembled by the building services contractor. Once the building is occupied, the accountable person is responsible for keeping it current as systems are replaced or modified.
What happens if the manual is out of date?
An out-of-date manual is a safety risk and an evidence gap: maintenance staff follow instructions for equipment that no longer exists. It should be brought up to date by reconciling the manual against the asset register and as-built records, and then version-controlled so it stays current.
Does the manual need to be digital?
Yes, in practice. The golden thread must be an electronic facility, and the manual's value comes from being findable, searchable and version-controlled. A scanned binder is a start; a structured, indexed manual set is what the regulator can actually use.
How does the manual connect to the safety case report?
The safety case report describes how safety systems are managed. The manual is the evidence of how those systems work and how they are maintained, so the two documents must cross-reference cleanly, which is another reason the manual must be current and structured.
